Our objectives were to report on the changes in the percutaneous technique and the role of endoscopy for delivering intrapulmonary therapies and, secondarily, to report the growing use of endoscopic techniques for delivering intrapulmonary treatments. Records from 11 centers were reviewed. From the total, 446 patients underwent 26,995 bronchoscopic procedures, and 108 patients received intrapulmonary therapies. Most patients (45%) were treated with endoscopically guided bronchial ablation for peripheral airway disease. Endoscopically directed bronchial blockers were most frequently used (95%) for patients with bronchial stenoses and neoplasms. Nasobronchial tubes were the predominant systems for tracheal stenting (78%) with the development of a new endoscopic stent and delivery system. A large number of patients received endoscopic therapy through percutaneous systems (47%). These procedures involved transtracheal jet ventilation with a dilator set, needle-type jet ventilation, and augmentation of the jet ventilation with saline jets and application of balloon dilators. Percutaneous delivery systems have been advanced in the delivery of intrapulmonary therapy; these include injection devices and techniques of endoscopic stenting. Endoscopically assisted interventional bronchoscopy has become an integral part of the management of intrapulmonary airway disease. Percutaneous intrapulmonary bronchoscopy is also an important minimally invasive technique for delivering intrapulmonary therapy.Help the Green Frog Triumph!
